A Clinical Study of Gall Stone



Abstract
Clinical review was carried out in 363 cases of gallstones, which had been admitted and treated at department of Surgery, Kyung Hee University Hospital, from January 1978 to December 1982.
The results were as follow:
1) The sex ratio was 1 male to 1. 3 female and the most prevalent age was 50-59 years. 2) The most prominent symptom was pain on right upper quadrant of abdomen, and the characteristic sign was the tenderness on same site.
3) The level of the serum bilirubin was normal range in 61.2% of cases, and in icteric group, mild jaundice (2~5mg%) was the most common.
4) Gall bladder was the most common site in which gallstones were located, and the majority of intrahepatic stones were located in both lobe of liver, simultaneously.
5) At the time of admission, 84.5% of cases were combined with complications, and the most common complication was the cholecystitis including gall bladder empyema.
6) Various techniques of operation were employed according to the location, nature, size, and the number of the stones. Cholecystectomy was most frequently performed. In the cases of intrahepatic stone, the basic operations were consisting of hepatectomy, transhepatic cholang-iolithotomy with U-tube insertion, hepaticoenterostomy, and extensive exploration of common bile duct with T-tube drainage.
7) The incidence of postoperative complication was 21.2%, and the most frequent incidence was residual stone.
8) Mortality rate was 3%.
